On June 9th we forwarded you a receipt form covering a cheque in the amount of $247.24 which we requested Treasury to mail to you direct. Since we do not appear to have received the completed form we are somewhat concerned as to whether or not the cheque has reached you. If you have received it and the original form has been lost or mislaid, we are enclosing herewith an additional copy for your convenience.

Your co-operation in returning the completed form to this Office would be greatly appreciated since it is necessary to complete our records.
